A rare case of a functioning retroperitoneal paraganglioma in a patient with recurrent Phaeochromocytoma/Paraganglioma (PPGL)

Hope David

Paragangliomas are rare neuroendrocrine tumours arising from extra-adrenal paraganglia of the autonomic nervous system and excess catecholamine secretion is associated with higher cardiovascular morbidity and mortality. We present the case of a 56-year old male referred to our outpatient clinic with symptoms of sympathetic-hyperactivity, including excessive sweating, palpitations and diastolic hypertension (140/100 mmHg). He had undergone...

Subclinical hyperthyroidism in pregnancy

Hope David

33 year old female who is 5 months pregnant, referred to Endocrinology clinic with TFT’s in keeping with sub clinical hyperthyroidism. Previously also had thyroid USS showing large 5 cm nodules both lobes, both indeterminate in appearance (U3) which preceded pregnancy. Symptomatic with sweating and palpitations. No family history of thyroid disease and no ‘red flag’ symptoms. Clinical examination showed palpable thyroid nodules with no evidence of thyrotoxicosis...

GLP-1/Glucagon dual agonist affects amino acid metabolism

Scott Rebecca , Hope David , Owen Bryn , Bloom Steve , Tan Tricia

Background: GLP-1/glucagon dual agonists are being developed as treatments for obesity due to their combined effect of reducing food intake while increase energy expenditure. Though the effect of the dual agonist on carbohydrate and lipid metabolism is well studied, little is known about the effects on protein metabolism. This study aimed to examine the acute and chronic effects of the GLP-1/glucagon dual agonist on amino acid metabolism. Methods: The lon...

A challenging case of hypercalcaemia due to primary hyperparathyroidism in an elderly patient successfully treated with non-elective surgical inpatient management

Hope David CD , Ostberg Julia E , Polazzo Fausto

An 86-year old man was admitted to hospital with gradual decline in mobility and leg weakness. Co-morbidities included atrial fibrillation, hypertension and osteoarthritis. The adjusted serum calcium on admission was 3.42 mmol/l; bone profile showed phosphate 0.56 mmol/l, alkaline phosphatase 140 U/l, albumin 34 g/l, parathyroid hormone 21.4 pmol/l. He was vitamin-D replete (68 nmol/l) with normal renal function. The hypercalcaemia was not thought to be due to any medications....

Challenges in management and pharmacotherapeutic strategy in patients with a prolactinoma and neuropsychosis; a case presentation and literature review

Moolla Ahmad , Hope David , Hill Neil , Meeran Karim

A 36-year-old male presented to the emergency department with a 24-hour history of mutism, paranoia and 3-day history of social withdrawal. He was unable to engage with healthcare professionals and the history was elicited from his mother. Due to a background of mild depression, an acute psychotic diagnosis was initially pursued. A CT scan on admission revealed a large pituitary lesion. A pituitary MRI scan thereafter demonstrated a large 25 × 30 × 18 mm sellar and s...

Nogueira Edson , Khin Kyaw , Hope David , Idowu Oluwagbemiga , Sathianathan Vivian , Darko Daniel , Rahman Mushtaqur

A 58-year old previously independent man with background of poorly-controlled hypothyroidism, T2DM, hypertension, ischaemic cardiomyopathy, and CKD presented to hospital feeling generally unwell, with a dry cough. His regular medications included anti-hypertensives, L-thyroxine 50 μg daily, linagliptin, insulin, aspirin, atorvastatin, and thiamine. On admission, the TSH was 83 mIU/l, free T4 6.5 pmol/l and free T3 was 1.9 pmol/l; four months earlier, the TS...

Background: Bariatric surgery is currently the most effective treatment for weight loss. Its metabolic effects of weight loss and improvement of type 2 diabetes are mediated mainly through the postprandial elevation of gut hormones such as GLP-1, which suppresses food intake and improves insulin secretion.Aim: To characterize the longitudinal changes in fasting and postprandial secretion of glucose, insulin and gut hormones (GLP-1 and GIP) in patients be...

Background: Roux-en-Y gastric bypass (RYGB) leads to effective and sustained weight loss whereas a Very-Low-Calorie diet (VLCD) is normally plagued by weight regain over time. Changes in gut hormones following each intervention may play a role. The aim of this study was to investigate gut hormone responses after 4 weeks of RYGB and VLCD.Methods: We prospectively recruited 21 volunteers with obesity and type 2 diabetes undergoing RYGB, and a matched cohor...
